National Health Plan Solution

Player’s Health is excited to invite you to participate in a national health insurance program.  By consolidating coaches (and their dependents) into a singular national health insurance program, we can offer richer health benefits at reduced rates.  Equally important, all participating coaches will have access to our customer service team to help you optimize your health plan benefits and help resolve service issues.  Watch the video from our founder to learn more.

Participation Options

Participation in the Health Plan is optional. In order to participate some information needs to be collected about your club and possibly the health plan participants.

For smaller clubs wanting individual insurance

Another option is for smaller soccer clubs that either have no group insurance and/or want to get out of group insurance and are interested in helping their coaches secure individual insurance. For these clubs, apply through the link below and select which individual insurance option works best for your club.

Club Information

For clubs looking to provide individual insurance, we only need information about your club. This should only take a few minutes to complete.

For large clubs with existing group insurnace

One participation option is for larger soccer clubs that have long-standing group insurance and are willing to help smaller groups by participating in a jumbo AHP that will eventually lead to better benefits and lower rates for all coaches.  For these clubs, we ask that you fill out our online survey, submit your coaches (and their dependents) names in the downloadable Excel spreadsheet and we will circle back with you with your group insurance rates and benefits.

Club Information

After you have prepared the census spreadsheet from Step 1, you're ready to complete the club survey. This should only take a few minutes to complete.

Key Dates

We are targeting a January 1st, 2022 kick off for our new national health plans. Given this timeline, it is important for us to keep things moving. We will be reaching out to all clubs in three phases. Below are the key dates we need to meet in order to achieve our January 1st implementation date.

Phase 1: Jul 13 - Jul 30

National Health Plan website and survey sent to all clubs in Phase 1. All club surveys and census files (info below) must be submitted by July 16th.

Phase 2: Aug 1 - Aug 20

National Health Plan website and survey sent to all clubs in Phase 2. All club surveys and census files (info below) must be submitted by August 6th.

Phase 3: Aug 23 - Sep 10

National Health Plan website and survey sent to all clubs in Phase 3. All club surveys and census files (info below) must be submitted by August 27th.

Oct 11, 2021

Rates will be presented to all clubs and interested clubs can opt-in.

Oct 18 - Nov 15

Open enrollment for those clubs interested in participating.

Jan 1, 2022

Health Plan start date for the 2022 plan year.

Participation Benefits

By offering this flexible approach (Group Insurance and Individual Insurance) under the same AHP solution, we ensure that all clubs of all sizes benefit from a singular customer service experience that includes:  

  • Automated electronic enrollment
  • Virtual open enrollment events
  • Ongoing year-long member service issue resolution support (claims/billing issues, prior authorization denials, member appeals, etc.)
  • Regular member engagement campaigns including ER avoidance (telemedicine adoption), preventive appointments, and health & wellness activation campaigns

Over time, as significant membership is achieved, a true AHP will be created using a singular group insurance plan. That is the uniqueness of our approach, the flexibility, phased approach with a common customer service portal.  We also invite you to speak with the organizer of the health insurance program.

What's Next?

After all interested member organizations have submitted their participant information with the completed club survey, the information will be provided to the insurance carrier for approval and health plan rates. After approval you will be contacted to complete your enrollment.
